New Patient Registration
If you live in our Practice area and would like to register with us, please complete one of our registration forms (GMS1) available from our reception.
Allocated Named GP
All patients registered with this practice will be allocated a named GP.
You can still make an appointment with any of the doctors in the practice.
Should you wish to know who your named doctor is, please ask the receptionist.
We are always happy to see any relatives or friends staying with you if they need a Doctor or Nurse. They will need to fill out a short form for the information to be returned to their own Doctor. Please arrive 10 minutes before your appointment to allow time for completion of the necessary forms.
We are a Training Practice
Sacriston Surgery is a ‘Training Practice’. This means Doctors wanting to enter General Practice spend six months with us in order to gain the experience they need to become Family Doctors. These are known as GP Registrars and F2 Doctors.
As a Training Practice, your medical records may be used for educational purposes. Because of this, we ask patients registering with us to sign a consent form giving permission for their records to be used in this way.
